Provide home-based care to patients with illnesses, injuries and chronic diseasesoften after hospital dischargeunder the supervision of an RNWork with a team of physicians, RNs, CNAs and aides to deliver quality patient careTake patient vitals, do assessments, document progress and report changes to RN supervisorAssist RN in carrying out patients plan of care and teaching patient self-care techniquesEducate family on the patients plan of care, medication and home safety Collaborate with interdisciplinary team to coordinate patient care and plan for dischargeA few must-haves for Home Health Licensed Practical Nurses:
Current Arizona LPN license in good standing.
One (1) year of recent clinical hands on experience.
Able to provide proof for current Basic Life Support (BLS) training including adults, children and infants.
Must be willing to travel to surrounding areas.
Home health experience preferred but not required.
